Event Description:
The issue has been resolved and tested as such, as of 1300h. The issue was resolved earlier, but our testing proved conclusive at around 1300h.

Inbound calls over toll-free numbers and numbers not originating with this carrier were not impacted. We continue to investigate, and are pursuing eliminating this carrier from our network.
The ultimate source of the issue seemed to be a top-tier interconnect this provider uses, it had become severely congested due to a suspected DDoS attack. We are still awaiting a detailed report from the carrier.
